Why does a print fail after several perfect hours?

Nothing hurts quite like checking a long print and discovering spaghetti after six hours. What are the most common reasons a print can start perfectly and fail much later?

Almost always one of: a spool tangle, a knock to the printer mid-print, or heat creep from a dusty heatsink. A partially clogged nozzle that finally gives up is also common.
