What does Z-offset actually change?

For beginners who keep hearing "adjust your Z-offset," what exactly is changing and what does a good first layer look like?

It sets the gap between nozzle and bed for the first layer, independent of physical leveling. A fraction of a millimeter changes everything: too high means spaghetti, too low means scraping. First-layer tuning IS Z-offset tuning.
